Dental Appointments

To make a dental appointment call 540-459-9333.

New patients without Medicaid will be asked for a $35 payment for an exam and x-rays, or for an emergency visit if required. This payment must be made in advance to schedule an appointment. All uninsured patients will be required to show proof of their Shenandoah County street address and recent pay stubs to assure they qualify for services.

Medicaid patients can give their card number when scheduling an appointment.

Medical Appointments

Appointment-PageTo make a medical appointment call 540-459-1700. The receptionist will ask a few questions to help assure that people qualify for clinic services or refer them to other resources if needed.

Eligibility for clinic services is evaluated on Wednesdays from 1-3 pm, and periodically thereafter. There will be NO PATIENT INTAKE on October 25, November 22, and December 27. Patients must show proof of their family’s total income, preferably three months of the most recent pay stubs, and their Shenandoah County street address at this appointment. The prior year’s tax filing (Form 1040) will be required for some services, so it is helpful to bring it if available. Patients are asked to budget a contribution of $10-$20 per visit. The amount is based on the patient’s household income. Patients who are unable to make this contribution at their visit are encouraged to talk with the staff as exceptions will be made on a case-by-case basis.
